What is Pre-IVF evaluation?

It is important that every patient is evaluated before starting the procedures of Assisted Reproductive Techniques or ART. This helps in maximizing the chances of victory. Determining preconception health is the first objective of this evaluation. A healthy and successful pregnancy is guaranteed after a firm start.
Couple tests
1. General health: Assessment of general health is the first step. Experts check the basic parameters like overall health, body weight, general fitness and stamina, presence of ailments like Diabetes, heart problems, high blood pressure of Thyroid disorders. Optimal body weight is suggested based on the physical structure.
2. Blood tests: Doctors perform various types of blood tests before starting IVF procedures. The tests reveal about immunity with diseases like Chicken Pox, Rubella, Hepatitis B, Hepatitis C, HIV and VD. If required, then specific genetic tests like cystic fibrosis, hemophilia and sickle cell can also be conducted.
Female tests
1. Ovarian Reserve Testing: Experts perform ovarian reserve testing to measure the quality and quantity of eggs so that reproductive potential can be determined. There are various test procedures to give accurate results:
A. Day 3 Levels of FSH, LH, and Estradiol: Levels of Follicle Stimulating Hormone (FSH), Luteinizing Hormone (LH), and Estradiol levels at the third day of menstruation give clarity about fertility potential. If the levels are elevated, then the success rate is quite low and a lot of medication may be required during the process.
B. Clomiphene Citrate Challenge Test: On the 3rd day of menstruation cycle FSH levels are measured. On 5th to 9th day of menstrual cycle, patients are given 100mg Clomiphene Citrate. Level of FSH is measured on the 10th day again. If there is significant increase in FSH level, then the pregnancy success rate could be low.
C. Anti-Mullerian Hormone Test (AMH): AMH levels give the number of eggs remaining in the ovaries. It can be performed on any day of the menstruation cycle.
2. Evaluation of the uterine cavity: It is important to evaluate uterine cavity because embryo will be placed ultimately in it. Doctors determine that it is normal by various tests. Hysterosalpingogram or hysteroscopy are the popular methods. They can be performed any time during the end of menstruation cycle and ovulation. Hysteroscopy is a process of looking at the cavity using a scope with a source of light. It shows the internal parts clearly and doctors can determine if there are any fibroids or polyps. HSG X-ray is another test that can give clarity about the overall health of the uterus.
Male tests
1. Semen testing: Doctors determine semen quality before starting IVF procedures. Semen analysis report throws enough light on the qualitative and quantitative parameters of sperm. Success of IVF largely depends on sperm quality.
Not all the above-mentioned tests are performed on each patient. Doctors decide the criticality and need of the respective procedures according to the case. One more important activity carried out before the actual treatment starts. Doctors perform a mock embryo transfer test to make sure that there is no irregularity in the cervix or uterus that may create problem during actual embryo transfer. In this test, a catheter is passed to the uterus via cervix. Angle and shape of the cervical canal and uterine depth are measured for further reference. A transvaginal sonogram confirms that the catheter has been placed properly. This test is conducted after menses and ovulation.
